Did Salesman Violate Campaign Finance Laws Too?

I suppose it's a deck chair on the Titanic, but check out this page of then-Miramar Commissioner Fitzroy Salesman's campaign finance treasurer's report from 2005. In the two boxes on the top, in the column that requires the name and address of the campaign contributor, Salesman wrote, "Fund Raiser" and the amount of $500. In the box below that, same phrase, with $225.

Of course, the "name" portion of the campaign finance disclosure is not optional. It pretty much defeats the purpose of "disclosure." As I understand it, the city clerk's office qualifies the campaign treasurers' reports. If so, why didn't they flag this?
